In this article we report the findings of a randomised control clinical trial that assessed the impact of a Philosophy for Children program and replicated a previous study conducted in Scotland by Topping and Trickey. A Cognitive Abilities Test was administered as a pretest and a posttest to randomly selected experimental groups and control groups. The students in the experimental group engaged in philosophy lessons in a setting of structured, collaborative inquiry in their language arts classes for one hour per (...) week for a number of weeks. The control group received the standard language arts curriculum in that one hour. The study found that the seventh grade students who had experienced the P4C program showed significant gains relative to those in the seventh grade control group at a high level of statistical significance, but the eighth grade students in the experimental group did not show such gains over the eighth grade control group. It was discovered that the seventh grade teachers started the program early in the school year and continued it for a period of 22 to 26 weeks, while the eighth grade teachers started much later and used the program for only 4 to 10 weeks. Our findings suggest that the P4C program must involve students in activities for a significant period of time before the program shows results, but that a meaningful impact on students’ cognitive abilities can be achieved in about 24 weeks of lessons, less than half the time evidenced by the study by Topping and Trickey. One major success of studying neural cell development in tissue culture has been the discovery of the O‐2A cell. This bipotential cell generates oligodendrocytes or, under certain conditions, a type of astrocyte. This essay considers the evidence that the characteristic properties demonstrated by the O‐2A cells in vitro are an accurate reflection of oligodendrocyte development in vivo.

Assessments of the acceptability of new transplantation practices require a pinpointing of not only the meaning of death, but also the timing of death. They typically perceive elective ventilation as occurring just prior to death and non-heart-beating donor protocols as operative just after death. However, such practices in fact highlight the general vagueness and ambiguity surrounding these issues in both law and ethics. Supply-side dilemmas in transplantation lend real urgency to this "life or death" debate.

This paper questions the dogmatic stance of the domestic courts toward mandatory orders for treatment, arguing that this has the potential to subjugate patients' interests to clinical discretion, and proposing a via media to accommodate the legitimate concerns of all parties.

Two contemporary strategies in cadaver organ transplantation, both with the potential to affect significantly expanding organ transplant waiting list sizes, have evolved: elective ventilation and use of nonheart-beating donors. Both are undergoing a period of critical review. It is not clear how widely EV is practiced around the world. In Great Britain, the Royal Devon and Exeter Hospital was the first hospital to develop an EV protocol, in 1988, after which other British hospitals followed suit. In the 1980s, new NHBD (...) protocols of two distinct types were implemented worldwide, although both rely on death confirmed by traditional cardiopulmonary criteria. The first type involves the removal of organs immediately after death, the preeminent example being the University of Pittsburgh Medical Center Protocol. The second involves the perfusion and cooling of kidneys immediately following death and subsequent organ removal. Protocols of this type have sprung up in Holland, Great Britain, Italy, France, Spain, Japan, and the United States. (shrink)

This article presents data from a follow up study done three years after the completion of the study reported in Fair et al.. The data show that the positive gains in scores on the Cognitive Abilities Test were (...) still present and had not faded after three years. Given the strength of these confirmations of the positive durable impact of the P4C program of structured discussions and given the relatively low cost of implementing the P4C program, it is recommended that it become a standard part of the school curriculum. Interoception can be broadly defined as the sense of signals originating within the body. As such, interoception is critical for our sense of embodiment, motivation, and well-being. And yet, despite its importance, interoception remains poorly understood within modern science. This paper reviews interdisciplinary perspectives on interoception, with the goal of presenting a unified perspective from diverse fields such as neuroscience, clinical practice, and contemplative studies. It is hoped that this integrative effort will advance our understanding of how interoception determines well-being, (...) and identify the central challenges to such understanding. To this end, we introduce an expanded taxonomy of interoceptive processes, arguing that many of these processes can be understood through an emerging predictive coding model for mind–body integration. The model, which describes the tension between expected and felt body sensation, parallels contemplative theories, and implicates interoception in a variety of affective and psychosomatic disorders. We conclude that maladaptive construal of bodily sensations may lie at the heart of many contemporary maladies, and that contemplative practices may attenuate these interpretative biases, restoring a person’s sense of presence and agency in the world. Who blows the whistle — a loner or a well-liked team player? Which of them is more likely to lead a successful opposition to perceived organizational wrongdoing? The potential influence of co-worker pressures to conform on whistle-blowing activity or the likely effects of whistle-blowing on the group have not been addressed. This paper presents a preliminary model of whistle-blowing as an act of nonconformity. One implication is that the success of an opposition will depend on the characteristics of the whistle-blower (...) and how the complaint is pursued. We develop the concept of a volitional disability as an aid in understanding those patients who behave in ways that are harmful to themselves in spite of their desire to do otherwise. Using this concept enables us to describe their behavior as intentional but ‘unvoluntary’. We demonstrate the clinical reality of such behavior by giving clinical examples of the behavior of those with phobic, compulsive, and addictive disorders. We then attempt to show how some kinds of self-harming behavior of noncompliant (...) patients are similar to phobic and compulsive behavior. We propose use of the concept of volitional disability to make it easier for physicians to work with these noncompliant patients and thus to improve their ability to provide better care for them. BackgroundJunior medical doctors have a key role in discussions and decisions about treatment and end-of-life care for people with dementia in hospital. Little is known about junior doctors’ decision-making processes when treating people with dementia who have advance care directives, or the factors that influence their decisions. To describe among junior doctors in relation to two hypothetical vignettes involving patients with dementia: their legal compliance and decision-making process related to treatment decisions; the factors influencing their clinical decision-making; and the factors (...) associated with accurate responses to one hypothetical vignette. MethodA cross-sectional survey of junior doctors, including trainees, interns, registrars and residents, on clinical rotation in five public hospitals located in one Australian state. The anonymous, investigator-developed survey was conducted between August 2018 and June 2019. Two hypothetical vignettes describing patients with dementia presenting to hospital with an ACD and either: bacterial pneumonia; or suspected stroke were presented in the survey. Participants were asked to indicate whether they would commence treatment, given the ACD instructions described in each vignette.ResultsOverall, 116 junior doctors responded. In Vignette 1, 58% of respondents selected the legally compliant option. Participants who chose the legally compliant option perceived ‘following patient wishes’ and ‘legal requirements to follow ACDs’ as equally important reasons for complying with the ACD. The most common reason for not selecting the legally compliant option in Vignette 1 was the ‘ACD is relevant in my decision-making process, but other factors are more relevant’. In Vignette 2, 72% of respondents indicated they would commence treatment and 18% selected they would not commence treatment.. Similar reasons influenced participant decision-making in Vignette 2, a less legally certain scenario.ConclusionsThere are critical gaps in junior doctors’ compliance with the law as it relates to the implementation of ACDs. Despite there being differences in relation to the legal answer and its certainty, clinical and ethical factors guided decision-making over and above the law in both vignettes. More education and training to guide junior doctors’ clinical decision-making and ensure compliance with the law is required. (shrink)

In a recent paper, Richard Rorty begins by telling us why pragmatists such as himself are inclined to identify truth with justification: ‘Pragmatists think that if something makes no difference to practice, it should make no difference to philosophy. This conviction makes them suspicious of the distinction between justification and truth, for that distinction makes no difference to my decisions about what to do.’ (1995, p. 19) Rorty goes on to discuss the claim, defended by Crispin Wright, that truth is (...) a normative constraint on assertion. He argues that this claim runs foul of this principle of no difference without a practical difference: ‘The need to justify our beliefs to ourselves and our fellow agents subjects us to norms, and obedience to these norms produces a behavioural pattern that we must detect in others before confidently attributing beliefs to them. But there seems to be no occasion to look for obedience to an additional norm – the commandment to seek the truth. For – to return to the pragmatist doubt with which I began – obedience to that commandment will produce no behaviour not produced by the need to offer justification.’ (1995, p. 26) Again, then, Rorty appeals to the claim that a commitment to a norm of truth rather than a norm of justification makes no behavioural difference. This is an empirical claim, testable in principle by comparing the behaviour of a community of realists (in Rorty’s sense) to that of a community of pragmatists. In my view, the experiment would show that the claim is unjustified, indeed false. I think that there is an important and widespread behavioural pattern that depends on the fact that speakers do take themselves to be subject to such an additional norm. Moreover, it is a.. (shrink)

The Bare Theory was offered by David Albert as a way of standing by the completeness of quantum mechanics in the face of the measurement problem. This paper surveys objections to the Bare Theory that recur in the literature: what will here be called the oddity objection, the coherence objection, and the context-of-the-universe objection. Critics usually take the Bare Theory to have unacceptably bizarre consequences, but to be free from internal contradiction. Bizarre consequences need not be decisive against the Bare (...) Theory, but a further objection—dubbed here the calibration objection—has been underestimated. This paper argues that the Bare Theory is not only odd but also inconsistent. We can imagine a successor to the Bare Theory—the Stripped Theory—which avoids the objections and fulfills the original promise of the Bare Theory, but at the cost of amplifying the bizarre consequences. In the first chapter of From Metaphysics to Ethics, Frank Jackson begins, as he puts it, ‘by explaining how serious metaphysics by its very nature raises the location problem.’ (1998, p. 1) He gives us two examples of location problems. The first concerns semantic properties, such as truth and reference: Some physical structures are true. For example, if I were to utter a token of the type ‘Grass is green’, the structure I would thereby bring into existence would be true (...) ... How are the semantic properties of the sentence related to the non-semantic properties of the sentence? Where, if anywhere, are the semantic properties of truth, content and reference to be found in the non-semantic, physical or naturalistic account of the sentence?’ (1998, p. 2) Jackson notes two possible answers to questions of this kind. This chapter is an edited transcript of a panel discussion at the Truth 20|20 Conference. The discussion centers around a discussion between P.F. Strawson and Gareth Evans recorded for the Open University in 1973. In the ensuing discussion, Strawson’s and Evans’ comments on truth are compared both to Ramsey’s work on truth just before his death, and also to contemporary pluralist accounts. One of the major themes of the discussion is the distinction, suggested by Strawson and Evans, between a ‘thin’ (...) notion of truth suitable for application across a variety of domains, and a more substantive notion appropriate for certain domains but not others. The panelists consider whether these two notions could be made to fit into a coherent overarching account, and whether the theory of truth is the right place to locate this ‘thin’/‘substantive’ distinction.
